Wilhelmina Margretta Kosmack died on 26 May 1897 at Renfrew, Renfrew County, Ontario, Canada, at age 22 according to Ontario Death Registration 012819. She was a clerk.2,13,8,14 The cause of her death was given as "inflamation" according to her death registration, and of general peritonitis according to her obituary.14 The informant on her death registration was her husband, Jacob Mayhew.8

     She was commonly knownas Minnie.15,16
The following information is from my personal research:
Note that Jacob Mahew's and Wilhelmina Margretta Kosmack's birth registrations are sequential and on the same date. This is not their dates of birth, but the date they were reported to the registrar. This is an interesting coincidence considering that they were later married.10

Minnie Kosmack of Admaston was listed as a witness to the marriage of Charles Brisco and Isabella Kosmack on 31 May 1892.17
The following obituary appeared in the newspaper the Renfrew Mercury published in Renfrew, Ontario 28 May 1897 :
Death of Mrs. Jacob Mayhew
Much sympathy is expressed for Mr Jacob Mayhew, in the death of his young wife. She was taken suddenly ill on Friday last with a violent attack of general peritonitis. Notwithstanding (sic) all that medical skill could do, she continued to sink and died on Tuesday at midnight. She leaves an infant four weeks old. Mrs. Mayhew was a daughter of Mr F. Kosmack.14

In the obituary of Ferdinand Kosmack who died 3 April 1912, her daughter, Mrs. J. Mayhew was listed as predeceased.18

He was born on 29 April 1897 at Renfrew, Horton Twp., Renfrew County, Ontario, Canada, according to Ontario Birth Registration 038038.5,3,2,4,6 The informant on his birth registration was Jacob Mayhew, his father.6
Louis Clinton Mayhew died on 25 September 1897 at Renfrew, Horton Twp., Renfrew County, Ontario, Canada, according to Ontario Death Registration 022841.3,7,8,9,1 The informant on his death registration was his father, Jacob Mayhew.1 The cause of his death was given as diarrhoea.1
In the obituary of Mrs. Jacob Mayhew who died 26 May 1897, his son, unnamed and referred to as "an infant four weeks old"., Louis Clinton Mayhew was listed as a survivor.10
The following analysis is from my personal research and reflects my attempts to consolidate information from different sources.
Diane Vermaat incorrectly reports (I believe) that Lewis Clinton Mayhew was the son of Jacob and Lucie?, and grandson of Joseph Mayhew and Marion Ferguson. She also records that Jacob and Minnie Kosmack had an unnamed son who died a week after his mother's death in childbirth. This unnamed son was Louis or Lewis Clinton Mayhew.11
