She was born on 2 May 1905 at Radcliffe Twp., Renfrew County, Ontario, Canada, according to Ontario Birth Registration 040262 Her parents were Ira Mayhew and Catheran (sic) Agnes Mahon. Her birth was registered Apr 18, 1906.3,1,5 She was baptized on 28 May 1905 at Our Lady of Holy Angels Church, Brudenell Twp., Renfrew County, Ontario, Canada. May 28th 1905 We the undersigned baptized Mary born on the second day of May of the lawful marriage of Ire Mayhew and Kate Mahon. The sponsors were Andrew Goyer and Mary Mahon. Francis L. French, P.P.7
Mary Fern Mayhew died on 25 October 1906 at at Lot 8, conc 3?, Radcliffe Twp., Renfrew County, Ontario, Canada, at age 1 according to Ontario Death Registration 026078. Her father's name was given as Noah Mayhew.4,1 The cause of her death was given as blood poisonng.1 The informant on her death registration was her cousin (and possibly her bioogical father), J. Mayhew.1
Mary Fern Mayhew was buried on 27 October 1906 at Our Lady of Holy Angels Cemetery, Brudenell Twp., Renfrew County, Ontario, Canada.8,9
     Research notes (September 2017):
Confusion reigns. In the parish record of her burial, her father is identified as Ira Mayhew and one of the withnesses was Joseph Mayhew. Are they the same person?

On her birth registration her father is Ira Mahew. On her death registration, her father is identified as Noah Mayhew by the informant, J Mayhew.

A still born daughter delivered Mar 1, 1905 was registered in Radcliffe with the father's name given as Noah Mayhew. This would be several months before Mary's birth.10

The following analysis is from my personal research and reflects my attempts to consolidate information from different sources.
Note that Mary Fern was born to Ira and Catherine Mayhew according to her birth registration, but on her death registration her father was given as Noah Mayhew.

I believe both of these documents refer to the same child, since Noah's daughter died at the stated age of 1 year and 5 months.

Ira was Noah's uncle. Did Ira and Catherine give up Fern to Noah and his wife Matilda? They had a stillborn child earlier in 1905.1,5
