He died on 17 April 1930 at Kankakee State Hospital, Kankakee, Kankakee County, Illinois, USA, at age 46. Note that his obituary incorrectly indicates he died in Chicago. He was a chauffeur.2,3,4
Guy Sivwright Maitland was buried on 22 April 1930 at Mayfield Congregational Cemetery (aka Mayfield Cemetery or Brush Point Cemetery), Mayfield Twp., DeKalb County, Illinois, USA.2,3,4,8

Census Data

Guy S. and Marie A. Maitland appeared on the 1920 U.S. Federal Census of Chicago, Cook County, Illinois, enumerated 2 January 1920, described as a chauffer for a cab company. Their children: Louella M., were listed as living with them.5
The following information is from my personal research at the book Past and Present of DeKalb County, Illinois recorded in Chicago, Illinois, 1907:
A long time resident of DeKalb County, George M. Sivwright came to the county when he was 7 years old. On Jan 1, 1860 he married Maria Louise Fairclo in Kingston, DeKalb County. Their only child, Guy T. Sivwright was born August 30, 1861. In the fall of 1883 he was injured in an accident on a skating rink, resulting in a spinal abcess. He died on June 15 of the following year.

Conclusion
Robert Maitland and his first wife must have been related to or at least knew and respected Guy, for they appear to have named their son Guy Sivwright Maitland after him. Their son was either born Nov. 23, 1883 around the time of the accident, or in Nov. 1884, after Guy T. Sivwright's death.9
Guy Sivwright Maitland, of Chicago, registered for the draft on 12 September 1918 at Chicago, Cook County, Illinois, USA, at the age of 34. He reported that he was an unemployed chauffer. He listed his next-of-kin/contact person as his wife, Marie Maitland. He was described as medium hiehgt and build with gray eyes and dark brown hair.1
The following obituary appeared in the newspaper The True Republican published in Sycamore, Illinois 23 April 1930:
Funeral in Mayfield This Tuesday of Guy S Maitland, Aged 46
Funeral services were held this Tuesday afternoon at 1:30 o'clock in the Mayfield church, Rev. R. H. Byler, officiating, over the remains of Guy S. Maitland. Interment was in the Mayfield Cemetery.
Guy S. Maitland died suddenly in Chicago on Wednesday, April 17, from paralysis from which ailment he had suffered for several years.
The son of Mr. and Mrs. Robert Maitland, Guy Maitland was born in Sycamore in November, 1884, and while yet a child his family moved to Iowa. In 1916 he was married in Chicago and had since that time resided there.
Besides his wife he is survived by three children, Guy, Jr., John Floyd and Luella, and by his father, Robert Maitland of Belmont, Ia., a brother, J. R. Maitland of Milwaukee and a sister Mrs. John Hyde of Garner, Ia.3

The following analysis is from my personal research and reflects my attempts to consolidate information from different sources.
His middle name is most clearer shown in his signature on his WWI draft registration.1
